With this is the necessity to provide support and services to people in their own home. Some tasks become more difficult or perhaps impossible and this is where Heart to Hearts offers a range of in-home services to make life easier. Our home support services try to involve our client as much as possible. We don´t take over and do everything. We recognize the need for our clients to maintain a degree of independence and where we can involve them in a task we will do so.

Home services allow our clients to maintain the freedom to live as normal a life as possible, which we believe is vitally important. We have built a number of relationships with diverse communities and are very proactive in providing services within those communities.

In addition to provide help within the home we recognize that our clients are often living alone with few or no family members close by. This can have a substantial impact on their lives, particularly in terms of mental health, that is why Heart to Hearts has developed a number of links with other services such as community programs where clients can meet other people and socialise and arrange events etc. We can link clients to access additional health services, accommodation services and equipment such as walking frames and other adaptive aids.

We are a provider that aims to work effectively and efficiently with My Aged Care and the National Disability Insurance Scheme (NDIS). This is a social welfare scheme funded by the Australian Government that funds costs associated with social care. The majority of our clients are eligible under this scheme.

We provide several services within the home environment of the client, including:-

  • Social Support through Community Projects
  • Respite and Companionship, a simple chat provides a lot
  • Welfare Health Checks.
  • Personal Care (including showering, dressing and toilet assistance)
  • Medication checks and assistance
  • Domestic Assistance (cleaning, ironing, Washing up etc)
  • Meal preparation (including nutritional advice)
  • Outside services (garden maintenance, lawn mowing)

At Heart to Hearts, we offer a broad range of services for our clients. Our services are creatively and holistically centered. We don´t believe in a one-size-fits-all policy. We treat all our clients as individuals and create a unique program for each one of them which reflects their specific needs. We serve many clients from ethnic minority communities and all our staff are diversity procedure trained and have a full understanding and awareness of cultural beliefs and are sensitive and fully supportive of all clients.

Heart to Hearts offers a full support package for those who require assistance in their own homes or for those who are no longer able to live at home. We can offer assistance with general household tasks, general health problems and mobility issues. We also offer links to other services including health care, assistance with accommodation and access to equipment such as walking frames and other adaptive aids. The majority of our clients are government funded and we can offer advice on how to apply for government assistance.

Although we are a non-medical/personal care provider we are a customer focused organization with a strong commitment to delivering a first class service delivered by dedicated, professional staff. Our service culture is at the core of our business with our clients being our number one priority at all times.

Our business provides a safe and supportive service for our participants. We are focused in engaging our clients in services that will help them to complete basic and simple task. It is not simply a case of doing everything, we always engage the client as much as possible in participating in all activities ensuring they feel independent and capable of making their own choices.

SOCIAL SUPPORT (COMMUNITY LINKAGE)

Meeting with other people in your community greatly improves your well-being both physically and mentally. We actively engage with participants, their families and carers together with the disability sector, advocacy groups, providers and other stakeholders to find social activities and events that may be of interest to them. Enabling them to meet new people and participate in a range of community activities, helping each participant to uncover hidden talent and skills.

We can provide a program of support to assist clients to boost their independence in terms of managing their lives. By working closely with our partnership group we can arrange contact with other providers and community groups giving you greater access to a whole range of community activities.

RESPITE AND COMPANIONSHIP

We fully understand the pressures of caring for a relative or friend who might otherwise require permanent help and support. That´s why our respite service is designed to give you a temporary break from caring.

We offer a flexible service which we can tailor to your needs. From a couple of hours off to a complete holiday break, one of our carers will step in allowing you to take a much needed break. All our carers are trained to care for clients who may need medical and physical support giving you peace of mind.

We know that living alone can sometimes be a lonely experience. Our in-home companionship service is a flexible program which can be arranged on a weekly basis with a varying timetable. Our companion carer will be there to help you with simple tasks and offer support or simply just a chat. This service is particularly useful for those who have just left hospital and need some support and help around the house whilst regaining their confidence and independence.

PERSONAL CARE

We offer help with personal care such as grooming (doing hair, applying make-up etc). Assistance with showering, toileting and dressing.

MEDICATION CHECKS AND ASSISTANCE

All our qualified staff have experience in administering medication. They can ensure that all our participants are taking the correct dosage of medication at the required time. Our staff can also liaise with doctors to report any problems and to arrange appointments.

MEAL PREPARATION AND NUTRITIONAL ADVICE

Our staff can assist with all types of meal preparation. We work with all our participants to create a meal plan/diary to ensure that they are eating a healthy balanced diet. We also offer advice on nutrition and can cater for all types of food allergies. We can advise on changes to diets and help implement them accordingly. Taking into account all types of budget restrictions.

DOMESTIC ASSISTANCE

Household chores can often become a burden for our clients. We offer assistance with a whole range of domestic duties including general cleaning, washing, washing-up, ironing, vacuuming. We can also help in the garden with lawn mowing, trimming, and general garden maintenance.

ADVANCED EXTRA RESPITE AND COMPANIONSHIP

We have now post COVID resumed our overseas trips. We hope to arrange soon more trips to Africa, Guided Safari Tours and The Village Life Camping experience.

All our trips are fully supervised by experienced staff. We are flexible with our tours and happy to cater for personal requirements wherever possible.

My Aged Care provides an entry point to Australian Government–funded Aged Care Home Packages/Services for the general public. THE NATIONAL DISABILITY INSURANCE SCHEME (NDIS) – WHAT IS THE NATIONAL DISABILITY INSURANCE SCHEME (NDIS)?

The NDIS scheme provides support and funding to any Australian under the age of 65 with any type of disability.
