We specialise in providing support and services to the elderly, those living with disabilities and/or Mental Health issues. Based in the South West region of Brisbane Heart to Hearts was founded on strong community roots and has a growing reputation for providing quality, caring support services to those that need it.
Our Mission

Our Services
We are a provider that aims to work effectively and efficiently with My Aged Care and the National Disability Insurance Scheme (NDIS). This is a social welfare scheme funded by the Australian Government that funds costs associated with social care. The majority of our clients are eligible under this scheme.
We provide several services within the home environment of the client, including:-
- Social Support through Community Projects
- Respite and Companionship, a simple chat provides a lot
- Welfare Health Checks.
- Personal Care (including showering, dressing and toilet assistance)
- Medication checks and assistance
- Domestic Assistance (cleaning, ironing, Washing up etc)
- Meal preparation (including nutritional advice)
- Outside services (garden maintenance, lawn mowing)

- To protect the rights of older Australians and those living with disabilities
- To offer the best support possible delivered in the way our clients/participants wants it
- To help clients/participants pursue and fulfill their goals
- To work closely with our clients/participants in the implementation of their plan
- To increase our clients/participants choices and independence
- To continue to develop the role of Heart to Hearts Community Services and improve it’s already outstanding service provision with the goal of making it as one of the best Aged Care Home Support Provider and NDIS provider in the area.

We are an Aged Care and Disability Service provider that work under My Aged Care Home Care Packages and the NDIS scheme.We provide disability support to clients in their homes under the scheme.
